What is Angiosarcoma?

Angiosarcoma is a rare and aggressive form of cancer that originates in the cells lining blood vessels. Poorly differentiated forms pose diagnostic challenges due to their heterogeneous appearance.

Immunohistochemical Markers

Immunohistochemistry aids in diagnosing poorly differentiated angiosarcoma:

Common endothelial markers include CD31, CD34, and VEGF. Among these, CD31 is the most reliable and sensitive marker.
Epithelioid angiosarcomas may express cytokeratin, complicating differentiation from poorly differentiated carcinomas.

Genetic Alterations

Research has uncovered genetic anomalies associated with angiosarcoma:

MYC Gene Amplification: Frequently found in secondary angiosarcomas, particularly those induced by radiation. Amplification is linked to UV and radiation exposure.

FLT4 Gene Alterations: Linked to lymphatic differentiation, FLT4 amplification is seen in about 25% of secondary angiosarcomas.

CIC Gene Rearrangements: Found in approximately 9% of primary cutaneous angiosarcomas.

PTPRB and PLCG1 Mutations: Identified as potential therapeutic targets due to their role in angiogenesis signaling.

Radiotherapy: High-energy radiation used to shrink tumors before surgery or destroy residual cancer cells afterward.
Chemotherapy: Anti-cancer drugs are used when surgery isn’t viable or if the cancer has spread.
Targeted and Experimental Therapies: Treatments like targeted therapy and immunotherapy are showing promise in clinical trials, though further research is needed.
After treatment, regular follow-ups are essential to monitor for recurrence. These may include:
Symptom discussions and physical exams. Imaging scans such as CT, MRI, or chest X-rays; Self-monitoring for local recurrence is also recommended.

Living with Angiosarcoma

Angiosarcoma can reappear in the same area (local recurrence) or spread to other parts of the body (metastasis), such as the lungs, lymph nodes, bones, liver, or brain. Prompt treatment is crucial in these cases.
